H&P Reimagined

Gather faster, present clearer.

Welcome to the FALCONNOTES Medical Data Miner collection. The vision of this collection is to help medical students transition from their pre-clinical years to their clinical years. As a medical student myself, I found that the overall variability between my preceptors and rotational experiences made it difficult to master the skills of 3rd year (obtaining a good history and physical, determining the problem, and presenting just the right amount of information to the attending). This, in turn, made it difficult to excel as a 3rd year (coming up with a plan, owning, and advocating for your patients, as they anxiously make their way through the healthcare system).

For this reason, we created a series of Medical Data Miners (MDM) to help students bridge the gap between their foundational knowledge and clinical acumen. We felt that it was critical not to make the mistake of producing one generalized template for every rotation that you embark on. Rather, we carefully designed each MDM to facilitate the unique learning that is expected to occur on each rotation.

What People Are Saying About MDMs

"This is a solid template for SOAP notes and it encompasses all the pertinent information that an EM attending or resident would ask from a medical student... Really wish I would have had something like this as a medical student rotating in the emergency department."

— Dr. Haseeb Khan, MD (Emergency Medicine Physician)

"This is pretty awesome! I think it's particularly a good idea for emergency medicine where the approach to patients and the time you have with them are both very different from most other settings and specialities...this is a great guide for anyone to start with. Something I would have loved to have during 3rd year."

— Dr. TJ Terlau, DO (Emergency Medicine Physician)

“Looks great, I don't think there's anything missing. Simple and minimal and just the right amount of information that a preceptor would be looking for. Flexible enough to get more information if you need to!”

— Dr. Ghufraan Akram, MD (Emergency Medicine Physician)
